How and where can I report unsafe food practices at a supermarket?

Dairy products are always spoiled upon purchasing (even though it's not close to the expiration date,) and frozen items have been totally defrosted and re-frozen. (Meaning, my pizza was smashed into a blob at the bottom of the box, but frozen.)
