Disorderly conduct, what to do with this paper?

I am 17 years old. I got a citation for disorderly conduct, in specific "the defendant, being less than 21 years of age, was found to be in possession of a bottle of vodka". on the back it says to send this form, with the dollar amount staed on the front (blah blah blah). but he did not write a dollar amount. he didnt tell me how much my fine is. so what exactly do i do with this paper? take it to the courthouse? mail it somehwere? i honestly am clueless. also, what is the average fines youve heard of for possession of alcohol by a minor? thanks!!
